Advanced Primary Care Management for Medicare Patients

At Novant Health, we are committed to giving you the care and support you deserve. That’s why we now offer Medicare’s Advanced Primary Care Management (APCM) program. This service provides more personalized attention, stronger care coordination, and 24/7 access to your care team.

What is Advanced Primary Care Management?

Medicare now covers Advanced Primary Care Management, which includes a range of services provided by your doctor.

When you enroll, you and your primary care provider (or care team) agree that they will be your main point of ongoing care. They’ll take the time to understand your needs, discuss your care plan, and coordinate your care across other doctors, hospitals and specialists. The program ensures that you always have a trusted team to turn to — whether it’s a quick question, a medication concern, or follow-up after a hospital stay.

  • Ongoing support to manage chronic conditions
  • Medication management and prescription refills
  • Coordinated care between specialists
  • Virtual care options, including e-visits and check-ins
  • Smooth care transitions, such as after a hospital stay

Who is Eligible for APCM?

Traditional Medicare and Medicare Advantage patients with a Novant Health primary care provider who offers APCM may be eligible. Your primary care provider can confirm if the program is available to you.

How do I sign up for this program?

Talk with your Novant Health primary care doctor during your visit or over the phone. You will give verbal or written consent for them to serve as your main point of care. This consent is recorded in your medical record. If you are eligible, they will guide you through enrollment and answer any questions.

How much does APCM cost?

For most patients, this is a fully covered benefit. Based on your insurance status, there may be a small cost.

If you have:

  • Medicare, you may be responsible for a 20% coinsurance (around $12 to $15 per month) after meeting your annual deductible.
  • Secondary insurance, it will cover the 20% Medicare coinsurance.
  • Medicare Advantage, your cost will depend on your plan — some may be $0, while others may have a small copay (same as what you pay when you see your primary care doctor for a sick visit).

You will not be billed separately for certain services like e-visits, clinician-to-clinician consults and patient advice requests.

How does this program help me manage my health?

APCM helps you stay connected to your care team, even between office visits. You’ll get a personalized care plan, regular check-ins, and support with medications, referrals, and follow-up after hospital stays.

Will I still see my other doctors?

Yes. You will still see your specialists and other doctors, but your Novant Health primary care doctor will serve as your main point of contact to coordinate all your care.

Can I stop the program if I change my mind?

Yes. You can stop APCM at any time. Just talk with your primary care doctor and they will update your record.
