Unpaid Leave
Novant Health offers the following unpaid leaves for when you need to be away from work. Novant Health partners with The Hartford to manage unpaid leave. You'll contact The Hartford directly to request a leave of absence.
Types of unpaid leave
Request a leave.
You, your leader, or a family member can contact The Hartford to request a leave of absence. The Hartford will collect information to determine eligibility and the type of leave you need. Based on the information collected, The Hartford will provide a certification or medical assessment form for you to have completed by your treating healthcare provider to support your request.
Apply for a leave by contacting The Hartford by phone or online.
Request to extend leave.
If you need to take more time away from work than expected, contact The Hartford to see if you're eligible for an extension or any other applicable leave. You will be required to update your provider documentation to determine if you’re eligible for additional leave.
Return to work.
You may begin the Novant Health return to work process by accessing the link below to complete the return to work request form. Please note that if you are returning from a leave of absence related to a personal medical condition (including maternity leave), you will need to provide a note from your treating healthcare provider as an attachment to the request which must include the following information:
- The date that you are cleared to return to work
- Any specific restrictions that you have related to your ability to perform the essential functions of your job with an end date for each of the restrictions indicated.
