Peripheral aneurysm repair

Your physician may recommend treatment based on the size, location and symptoms of your aneurysm. Common reasons include: 

• A popliteal artery aneurysm found behind the knee, which can affect circulation to the lower leg
• A femoral artery aneurysm in the groin area
• An iliac artery aneurysm found in the pelvis
• Aneurysms affecting the renal or other visceral arteries that supply organs such as the kidneys or intestines
• Aneurysms that are growing in size over time
• Symptoms such as pain, swelling or a pulsing lump
• Signs of reduced blood flow to the leg or affected organ
• Blood clots that have formed within the aneurysm 

Your specialist will use imaging tests to confirm the location and size of the aneurysm before recommending a treatment plan. 

Treatment depends on the artery involved, the aneurysm's size and your overall health. Options may include: 

Nonsurgical 

• Monitoring: For small aneurysms that aren't causing symptoms, your care team may recommend regular imaging and follow-up visits to watch for any change. This approach is common for smaller iliac or visceral artery aneurysms.
• Embolization: A minimally invasive procedure that blocks blood flow into the aneurysm using tiny coils or other materials. It's often used for renal and other visceral artery aneurysms, sometimes alongside a stent graft.  

Surgical 

• Endovascular repair with a stent graft: A minimally invasive procedure that guides a fabric-lined mesh tube through a small incision, usually in the groin. The stent graft lines the artery from the inside and seals off the aneurysm so blood flows safely through. This is often an option for femoral, iliac and some popliteal aneurysms.
• Open surgical repair: Your surgeon removes or bypasses the weakened section of artery and restores healthy blood flow, often with a graft. This is a common approach for popliteal aneurysms and for larger or more complex aneurysms. 

Popliteal artery aneurysms often require prompt evaluation because of their association with blood clots and reduced circulation to the leg. Iliac artery aneurysms may be monitored or treated depending on their size and whether they're causing symptoms.

Recovery depends on the type of repair you have and the artery involved. 

• Monitoring: No recovery time is needed, but you'll continue regular follow-up appointments and imaging
• Embolization: Recovery is typically brief, with most patients resuming normal activities within a short time, guided by their care team
• Endovascular repair: Many patients go home within a day or two and return to light activity within a couple of weeks, with follow-up imaging to confirm the repair is holding
• Open surgical repair: Recovery takes longer and may involve a hospital stay of several days, followed by weeks of gradually increasing activity 

Your care team will give you specific guidance on activity restrictions, wound care and follow-up visits based on your procedure. 

As with any vascular procedure, peripheral aneurysm repair carries some risks, including bleeding, infection, blood clots or damage to nearby blood vessels or nerves. Endovascular repairs may carry a risk of the stent graft shifting over time, which is why follow-up imaging is important. Open surgical repair involves the general risks associated with major surgery, including a longer recovery period.
