Your medical record contains information on your health history, and the care and treatment you received during your stay with us.
The medical record is the property of the hospital and it must be completed by all physicians involved in your care before it can be copied and released to the patient. Therefore, you may not receive a copy of your medical record upon discharge from the hospital.
Patient-related information may be released to a physician or medical facility for follow-up and continued care of the patient when needed.
Your medical record is a confidential document, so your written authorization is required before a copy can be released. If you're age 18 or older, you can get an authorization form from the medical records office or call 704-384-4150. A fee may be required to provide your records.
