Social Work Internship at Novant Health

You will report to/receive assignments from:

• Office of Health Equity & Community Health
• Director
• Program supervisor
• Ambulatory clinics

Your supervisor may not always be on site; some supervision could be virtual.

Your primary responsibility is to deliver a remarkable patient experience and understand your role in advancing our commitment to patients and families. Duties focus specifically on supporting vulnerable populations and addressing social drivers of health. This includes connecting patients to culturally and linguistically appropriate care, education and resources in both clinical and community settings — in homes, clinics and shelters. You will also provide brief therapeutic interventions and crisis intervention to patients in need.

You will collaborate with care teams and community partners to assess needs, assist with care planning, and support for applications for public benefits and community resources. Through outreach and coordination, you will help bridge gaps between patients, providers and support systems.

• Explore the impact of social work in both medical and community settings.
• Embrace teamwork and learn how community health workers, interdisciplinary medical teams, and local resources support patient outcomes.
• Maintain ongoing patient relationships and ensure continuity of care through in-person and phone follow-ups.
• Help patients navigate community and organizational resources that support their health and well-being.
• Share relevant updates with care teams — with patient consent.
• Build strong, supportive connections with care team members and community agencies.
• Participate in regular supervision to discuss areas of growth or concerns.

• Complete screenings for depression and anxiety, crisis intervention and safety planning.
• Assess patient needs, barriers and support systems.
• Assist with care planning to improve health and social outcomes.
• Complete documentation accurately and confidentially.

Uphold professional accountability by protecting patient privacy and confidentiality, advocating for patient health, and adhering to ethical, legal and regulatory standards.

• Participate in continuing education focused on electronic medical record systems, community resources, policies and procedures, social drivers of health, and care innovations for the most vulnerable.
• Foster a sense of belonging and connection among fellow interns.
