Clinical Pharmacist Practitioner (CPP)

Your CPP is a specially trained pharmacist who works closely with your care team to help manage your medications and keep your treatment plan simple and effective. Your CPP can help you:

Review all of your medicines to make sure they are safe and working well together
Adjust doses, start or stop certain medications under your provider’s guidance
Help manage conditions like diabetes, high blood pressure, high cholesterol, and others
Answer your questions about side effects, costs, and how to take your medicines correctly

Medication Management Specialist (MMS)

Your MMS helps make managing your medications simpler, more reliable and easier to fit into your everyday life. They can:

Check in about missed doses or delayed refills
Set up 90-day refills, automatic refills or mail-order options when available
Work with your pharmacy and insurance on needs like prior authorizations or high copays
Help you build routines, reminders or pillbox systems that fit your lifestyle

Patient Service Coordinator (PSC)

Your Patient Service Coordinator is your go-to resource for scheduling and general questions about the Mediful program. They keep everything organized so you always know who to contact and what to expect next. Rely on them to:

Help you schedule or reschedule phone or video appointments
Connect you with the right team member, such as a CPP or MMS
Assist with paperwork, enrollment and follow-up steps
Make sure your questions reach the right person and you get timely answers

How We Work Together for You

Get ready for your first visit.

Your first appointment with our clinical pharmacist practitioner will be a video visit. To schedule a virtual health care visit with your provider, call your provider’s office directly or use MyChart. A secure link will be sent to you within 24 hours of your appointment.

Video visits are an easy way to see your provider whenever your provider's office is open. To help you get the most of your time, here is a short list of things to prepare for you virtual visit.

Confirm whether your computer, phone or tablet has video capabilities and enough battery life remaining to complete the visit
- Please connect through MyChart prior to appointment time
- If you do not have MyChart, sign up for the easy-to-use app keeps you connected to your care team
You will need to make sure you have downloaded the Zoom Workplace app to your phone, tablet or computer so you can test your device an ensure it is compatible with the virtual care software
Find a quiet place that you are comfortable discussing your medical information
Have a list of medications or have your medication bottles ready to review at time of visit
If you check your blood sugar or blood pressure, have that information available for review at time of visit
