Appointments

We do not require a referral in order to be seen. We serve adults with orthopedic injuries and conditions, as well as children with orthopedic injuries. Call us during office hours to schedule your visit.

If you have had surgery previously, we will review your medical records before scheduling to ensure you get the care you need.

New Patients

After you schedule your first visit, you will receive information about completing new patient paperwork online.

We ask that new patients arrive 15 minutes before their first scheduled appointment so we can get any necessary information.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous imaging or recent medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
