Appointments

We see adults for all orthopedic injuries and conditions, as well as injured children. You do not need a referral to be seen. To schedule a visit, please call us during office hours.

If you have had surgery previously, we will need to review your medical records prior to scheduling to ensure you get the care you need.

New Patients

You will receive instructions about completing new patient paperwork online before your first appointment.

So that we can get your provider any needed information prior to your visit, we ask that you arrive at least 15 minutes early.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous imaging or recent medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
