Appointments

Our clinic does not require a referral. We servee adults for all orthopedic injuries and conditions, as well as injured children. Call us during office hours to schedule an appointment.

If you have had surgery in the past, we will need to review your medical records prior to scheduling to ensure you get the care you need.

New Patients

We will give you instructions to complete new patient paperwork online before you see us. So that we can collect necessary information from you, please arrive 15 minutes before your first visit.

What to bring

• Your photo ID
• Your insurance card, or your child’s insurance card if we are seeing your child
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous imaging or recent medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
