Appointments
We are a referral-based clinic. To see a specialist in our clinic, please have the provider involved in your care make a referral. We will call you to schedule your visit once we receive your referral information.
If you are an established patient, you can use MyChart to schedule follow-up care online anytime, or you can call us during office hours.
New Patients
We look forward to seeing you! Please arrive at least 15 minutes before your first visit so we can collect necessary information from you.
What to bring
Please arrive early and bring the following items with you:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
