Review the information below for important notes about what to bring with you to your appointments, and what to expect when you arrive. We have also provided patient information forms for your convenience; completing these at home before your appointment can help expedite your visit with us.

What to bring

Bring your insurance cards and your copay/coinsurance to every appointment. Also bring a complete list of medications, including vitamins, herbal supplements or over-the-counter remedies, along information about your dosages and frequency of use. We will also ask you to provide information about your medical and surgical history, recent X-rays and other pertinent medical records.

New patients

Please arrive 15 minutes prior to your appointment to complete the required administrative tasks.
