Appointments
To schedule an appointment with a provider in our clinic, use MyChart anytime or open scheduling through our provider page online anytime. You can also call our clinic during office hours so we can schedule an appointment for you.
Should you need to cancel or reschedule, kindly call us 24 hours in advance.
What to bring
We look forward to serving you. While our clinic does not require a referral, your insurer might. Please confirm insurance coverage before scheduling your appointment.
Arrive at least 15 minutes before your first visit. Bring with you:
- Your photo ID
- Your insurance card
- Your copayment or payment in full if insurance will not cover your visit
- A list of medications you are currently taking, including dosage
- Forms you have completed ahead of time using the links below
