Appointments
In order to see a provider in our clinic, you will need a referral from your Novant Health primary care provider. Please contact your provider and ask to be referred to our clinic.
To schedule follow-up visits, please call our office.
New patients
Please arrive at least 15 minutes before appointment time. Bring the forms you have been given to complete ahead of time. You can also use the links on our site to download and complete your forms.
What to bring
- Your photo identification card
- Your insurance card
- Your method of copayment, or payment in full if your visit will not be covered by an insurance plan
- Medications you are currently taking
- Forms you have completed ahead of time
