Appointments
We look forward to welcoming you each time you visit Novant Health Psychiatry – Concord. You can schedule your visit online using MyChart, or call our office during business hours. Please arrive at least 15 minutes early to help us serve all guests efficiently. Call our office at least 24 hours in advance if you need to reschedule or cancel an appointment.
What to bring
Please bring your insurance card and an ID card with your photo. Also bring a list of your current prescriptions and over-the-counter medications. Your copay is due on the day of your visit. Payment in full is due if you are not covered by a health insurance plan.
New patients
We look forward to helping you create a personal plan for better health. Before calling our office to schedule a visit, contact your Novant Health primary care physician and ask for a referral. This will help ensure we have access to your medical records. New patient visits are 60 minutes. Please arrive at least 15 minutes early on your appointment day.
Patient forms
The forms provided here are for new patients. Print the forms, fill them out and bring them with you for your visit. You also have the option to arrive early to fill out the forms in our office, or you can ask to have the forms sent to you by mail.
