Appointments

So that we can gather necessary information before your first visit, we ask that you get a referral from your primary care provider or another provider involved in your care. We will contact you to schedule your appointment after we receive that referral.

If you are an existing patient or need to speak with our team, please call our office. You can also call our office during office hours and we will schedule an appointment for you.

Our office is closed on federal holidays. Please call our office to confirm availability or speak to our team with any scheduling concerns.

If you ever feel you might harm yourself or someone else, or you are concerned a loved one could harm himself or someone else, call 911 or head to your nearest emergency room. You can also contact our Outpatient Assessment Center at 800-718-3550.

New patients

Please arrive at least 30 minutes before your first appointment. Understand that your first appointment could be lengthy as we will complete a full evaluation. Bring with you:

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • Any medications you are currently taking
  • Any forms you have completed ahead of time using the links below. These forms will also be available in our office.
