Appointments

So that we can have necessary information about your child before your first visit, we request that you ask your child’s primary care provider or another provider involved in her care for a referral to our clinic. Once we receive that referral, we will contact you to schedule your first appointment.

If you are an existing patient or need to speak with our team, please call our office. You can also call us during office hours and we will be happy to schedule an appointment for you.

If you ever feel you might harm yourself or someone else, or you are concerned a loved one could harm himself or someone else, call 911 or head to your nearest emergency room. You can also contact our Outpatient Assessment Center at 800-718-3550.

New patients

Please arrive at least 30 minutes before your child’s first visit. Understand that this visit might be lengthy as we complete a full evaluation. A parent or guardian who is able to provide health and behavioral information will need to remain in our office for the entire visit.

Bring with you:

What to bring

  • Your photo identification
  • The insurance card covering your child
  • Your method of copayment or payment in full if your child’s visit will not be covered by insurance
  • Any medications your child is currently taking, including any over-the-counter medications or supplements
