To make an appointment to see a provider in our clinic, use MyChart or open scheduling on our provider page. You can also call us during office hours and we will be happy to schedule an appointment for you.

If you need to cancel or reschedule an appointment, please call at least 24 hours in advance. Scheduling your next visit will be delayed if you fail to cancel at least 24 hours in advance, and you will need to pay a missed appointment fee before your visit can be rescheduled.

Attending appointments is crucial to your care. Missing or canceling three appointments can result in dismissal from our clinic.

New patients

Please arrive at least 30 minutes before your first visit so you can complete necessary forms. If you choose to download and complete forms ahead of time using the links below, you will need to arrive 15 minutes early. Bring with you to every visit:

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • All medications you are currently taking in their original bottles
  • Any forms you have completed ahead of time using the links below
