Appointments
You can make an appointment by calling our office during normal business hours or through MyChart.
What to bring
We ask our patients to bring the following to each of their visits:
- Your insurance card and form of identification.
- Your method of payment or copayment.
- Specific information on current medications you are taking including any herbal supplements or over-the-counter medications.
We look forward to seeing you in our office! We request that all patients arrive 15 minutes prior to your scheduled appointment to ensure paperwork is complete. Please have any records sent from your previous office prior to your first visit.

- Patient Information/Consent to Treat
- Authorization to Disclose Protected Health or Billing Information
- Acknowledgement of Receipt of Notice of Privacy Practices
- Communicating Your Health Information
For well-child visits for patient’s ages 2 months to 5 years, please download, complete and bring with you the Survey of Well-being of Young Children. This will help your child's provider understand his or her health and development. Click here to download the age-specific survey.
