We look forward to taking care of your needs at our clinic. Returning patients may use MyChart to schedule a visit online anytime. You can also call our clinic during office hours, and we will schedule a visit for you.
New patients
We welcome new patients to our clinic. Please arrive at least 15 minutes before your scheduled appointment so we can collect necessary information from you.
What to bring
- Your photo ID (such as a driver’s license)
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- List of current medications and supplements you are currently taking, including name and dosage
