Appointments
We look forward to seeing your child in our clinic. You can use MyChart anytime to schedule an appointment. You can also call us between 8 a.m. and 4:45 p.m., and our team will be happy to schedule a visit for you.
What to bring
New patients should arrive at least 15 minutes before your first visit so we can collect necessary information. You can also use the links below to complete required forms ahead of time. Bring:
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any forms (for a school, sports or camp physical) that you need our office to complete
- Any forms you have completed ahead of time using the links below
