Appointments
To make an appointment with a provider in our clinic, call us during office hours. You can also schedule online anytime using MyChart or through our physician finder.
Schedule your first visit with or without a referral from your primary care provider. Your insurance plan may cover the cost of your visit. Our team will verify your coverage.
What to bring
We look forward to seeing your child in our office. Please arrive at least 15 minutes before your scheduled appointment. You can download and complete necessary forms ahead of time using the links below. These forms will also be available in our office. Bring with you:
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if your child’s visit will not be covered by insurance
- Forms you need completed for school or sports participation
- Medications your child is currently taking or orthopedic devices your child is currently using
