Appointments
In order for us to get necessary information about your child’s medical history before you visit us, please have a provider already involved in your child’s care make a referral. We will contact you to schedule a visit once we receive referral information.
If your child is an established patient and needs follow-up care, you can schedule through MyChart or by calling us during office hours.
Please call us if you are seeking a second opinion.
New Patients
We look forward to serving you. We ask that you arrive 15 minutes before your child’s first scheduled appointment.
What to bring
- Your photo ID
- The insurance card covering your child
- Your method of copayment or payment in full if insurance will not cover your child’s visit
- A list of any medications your child is currently taking, including name and dosage
- Any recent scans or test results you have in your possession that are not in the Novant Health system
- Any forms you need our office to complete
- Any forms you have completed ahead of time using the links below
