Appointments
To schedule an appointment for your child with a provider in our office, call us during office hours, or use MyChart or open scheduling through our physician finder to schedule online anytime.
We look forward to seeing you and your child in our office. Please arrive at least 15 minutes before your first appointment so we can collect necessary information. Bring with you:
- Your photo ID card
- The insurance card covering your child
- Your method of copayment or payment in full if your child’s visit will not be covered by insurance
- Medications your child is currently taking, including over-the-counter medicines
- Recent test results you have been given that we might not have access to in our office
- Forms you need to have completed in our office (including school, camp or sports forms)
