Appointments

If you’d like to schedule a visit for your child at our office, please ask your child’s pediatrician for a referral. You can also schedule a visit with a member of our team through open scheduling on the physician finder. 

If this is your first time at our clinic, please arrive at least 20 minutes early. This should give you enough time to complete important paperwork before your child’s visit.

What to bring

• Your photo ID
• A method of payment to cover the cost of your child’s visit
• The insurance card that covers your child
• A list of the medications your child takes on a regular basis
