Appointments
You can schedule an appointment with us online anytime using MyChart or by calling us during clinic hours.
If you need to be seen quickly because of an injury or other urgent need, please call us to see how we can help.
If your insurance requires a referral before making an appointment, please have your doctor send us a referral and we will call you back to schedule an appointment once we receive it.
New Patients
Please arrive at least 15 minutes before your scheduled appointment. Arriving early gives you time to complete any necessary paperwork.
What to bring
- Photo ID (such as a driver’s license)
- Your insurance card
- Your method of copayment or payment in full if your visit will not be covered by insurance
- Recent scans or test results
- List of current medications and supplements you are taking, including name and dosage
