Appointments
To see a provider in our clinic, you can schedule online anytime using MyChart. You can also use open scheduling through our Providers page, or call us during clinic hours so we can schedule a visit for you.
Our clinic does not require a referral, but your insurance might. Please confirm insurance coverage before scheduling your visit.
New Patients
We welcome new patients! Please arrive at least 15 minutes before your first visit so we can collect necessary information from you.
What to bring
• Your photo ID
• Your insurance card
• Your method of copayment or payment in full if insurance will not cover your visit
• Any recent imaging or test results from outside the Novant Health system
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
• Any forms you have completed ahead of time using the links below
