Scheduling an appointment with Novant Health Orthopedics & Sports Medicine – Cornelius is as simple as calling us during our office hours. You can also schedule online anytime using MyChart or through our provider page.

Please arrive at least 15 minutes before your first appointment. You can download and complete necessary forms before your visit using the links below. Bring to your appointment:

What to bring

• Your photo ID
• Your insurance card or the insurance card covering your child if your child is the patient
• Your method of payment for your copay, or payment in full if your visit will not be covered by an insurance plan
• Recent scans or test results you have in your possession
• Medications you are currently taking (or your child is currently taking if your child is the patient)
