Appointments
Please ask your primary care doctor for a referral to our clinic. Depending on your insurance plan, you may be permitted to schedule your visit without a referral. You can use MyChart to make an appointment using a smartphone or computer if you have previously received care with us.
New patients
We ask that you come in 15 minutes early for your first visit with us. This will give you time to fill out important forms that will help us better serve you. If you have scans or X-rays from your visits to other medical facilities, please bring those with you.
What to bring
- Your photo ID
- Your insurance card
- A list of the medications and supplements you are taking, including dosage information
- A method of copayment or full payment, if this visit will not be covered by insurance
