Appointments
While we do not require a referral in order for you to be seen, your insurance might require a referral in order for your visit to be covered. Please confirm coverage before scheduling.
You can make an appointment by calling us during office hours. Established patients can also schedule follow-up care online using the patient portal.
New Patients
We look forward to serving you. Once you have scheduled your first appointment, you will receive information about filling out new patient paperwork before your visit. We ask that you arrive at least 15 minutes before your first scheduled appointment.
What to bring
- Your photo ID
- Your insurance card, or your child’s insurance card if we are seeing your child
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
