Appointments
We look forward to seeing you at our office. To schedule a visit for yourself or your child, please call our clinic during office hours. You can also use your MyChart account to make an appointment.
If this will be your first time at our office, please arrive at least 15 minutes early. This will give you enough time to finish paperwork for yourself or your child before seeing the doctor.
What to bring
- A list of medications or supplements that you or your child regularly take
- Your insurance card, or the insurance card that covers your child
- A form of photo ID
- A method of payment to cover the cost of your appointment, or your child’s appointment
