Appointments
If you are interested in receiving care at our clinic, speak with your primary care provider about a referral. Returning patients can conveniently schedule appointments through their MyChart account.
New patients
We ask that new patients arrive at least 15 minutes before for their appointment. Arriving early gives you time to complete any necessary paperwork.
What to bring
- Insurance card
- Photo ID (such as a driver’s license)
- List of current medications and supplements
- Form of payment
