Appointments
A referral from your primary care or other care physician is required to schedule an appointment. Once received, someone from our clinic will contact you directly to schedule an appointment. Please have information about your health insurance plan available so we can help you determine coverage for your visit.
You will receive a reminder call from our clinic one to two days before your appointment. If you need to reschedule or cancel your appointment, please call our clinic 24 hours in advance. If you call after our normal business hours, your call will be answered by our after-hours service.
What to bring to an appointment
Please bring your insurance cards, as well as a list of your current prescriptions and over-the-counter medications, including herbal or nutritional supplements. Please include information about doses and frequency of use so that our team has a good understanding of your medication routine.
