Appointments

Our clinic does not require a referral before scheduling an appointment, but your insurer might require one for your visit to be covered. Please confirm coverage before scheduling your visit.

New Patients

We look forward to serving you. Before seeing a provider in our clinic, we will need records from any cardiologists you have previously seen.

When you schedule your first appointment, you will receive information about completing new patient paperwork before your visit.

We ask that you arrive 15 minutes before your first appointment so we can collect any needed information.

What to bring

• Your photo ID
• Your insurance card
• Your method of copayment or payment in full if insurance will not cover your visit
• Any previous cardiology-related medical records you have in your possession
• A list of any medications you are currently taking, including name and dosage
• Any forms that you need our office to complete
