You need a referral from your primary care provider to be seen in this clinic. You can find one on our Providers Page.

Find a Primary Care Provider

To see a provider in our clinic, please ask a provider already involved in your care to make a referral. Once we receive your referral information, we will contact you to schedule your visit.

To schedule follow-up care, please call us during office hours.

New Patients

Please arrive 15 minutes before your first visit. This will allow us to collect necessary information before you see a provider.

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • A list of any medications you are currently taking, in their original containers
  • Any forms you have completed ahead of time using the links below
