You need a referral from your primary care provider to be seen in this clinic. You can find one on our Providers Page.

We look forward to serving you in our clinic. To make an appointment, please call us during office hours so we can schedule one for you. This allows us to make sure adequate time is set aside for you to see your provider.

If you will be receiving a diagnostic procedure, do not wear any lotions or oils as those could affect the test.

New Patients

Please arrive at least 15 minutes before your first visit so we can collect necessary information.

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • A list of any medications you are currently taking, including name and dosage
  • Any forms you have completed ahead of time using the links below
