You need a referral from your primary care provider to be seen in this clinic. You can find one on our Providers Page.

Find a Primary Care Provider

So that we can make sure we are prepared for your visit and you get the time you need with a provider, we ask that you call our clinic during office hours so we can make your appointment.

If you will be having a diagnostic procedure, please do not wear lotions or oils as those could affect tests.

New Patients

Please arrive at least 15 minutes before your first visit so we can collect necessary information from you.

What to bring

  • Your photo ID
  • Your insurance card
  • Your method of copayment or payment in full if insurance will not cover your visit
  • A list of any medications you are currently taking, including name and dosage
  • Any forms you have completed ahead of time using the links below
