We look forward to seeing you in our clinic. If you are a returning patient, you can use MyChart or open scheduling through our provider page to schedule your next visit, or call our office to during business hours for assistance.
What to bring
The providers at Novant Health Forsyth Heart & Wellness see patients based on referrals. If you would like to see a provider in our office, please ask your primary care provider or a specialist involved in your care to make a referral. Once we receive that referral and information about your condition, we will contact you to schedule your first appointment.
Please arrive at least 15 minutes before your first appointment, and bring:
- Your photo ID
- Your insurance card
- Your method of payment to cover your copay, or payment in full if your visit will not be covered by an insurance plan
- A list of medications you are or your child is currently taking
- Forms you have completed ahead of time using the links below
