Appointments
You do not need a referral to see a provider in our clinic. However, your insurer might require a referral in order for your visit to be covered. Please check with your insurer prior to scheduling.
MyChart is available to established patients to schedule an appointment online anytime. Established patients can also call our clinic during office hours so a team member can schedule a visit for you.
If you will miss your appointment for any reason, please let us know beforehand so we can offer your appointment time to another patient. You might be asked to reschedule if you arrive after your scheduled appointment time.
New Patients
We look forward to seeing you for your first visit. Please arrive at least 15 minutes before your appointment.
What to Bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- Any medications you are currently taking, in their original containers, including over-the-counter medications, vitamins and supplements
- Any forms you have completed ahead of time using the links below
