Appointments
Our clinic does not require a referral in order for you to see a provider, but your insurance might require one. Please check with your insurer before scheduling.
Existing patients can use MyChart to schedule online anytime, or can call our clinic during office hours so we can schedule a visit for you.
If you are unable to make your appointment, kindly let us know so we can offer the time to another member of our community. If you arrive after your appointment time, you might be asked to reschedule.
New Patients
We welcome new patients in our clinic! Please arrive at least 15 minutes before your first visit.
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- Any medications you are currently taking, in their original containers, including over-the-counter medications, vitamins and supplements
- Any forms you have completed ahead of time using the links below
