Appointments

To schedule an appointment with our clinic, please call us during clinic hours. Returning patients can schedule an appointment with us online anytime using MyChart. 

If your insurance requires a referral before making an appointment, please have your doctor send us a referral and we will call you back to schedule an appointment once we receive it.

New Patients

We welcome new patients. Please arrive at least 15 minutes before your scheduled appointment. Arriving early gives you time to complete any necessary paperwork.

What to bring

• Photo ID (such as a driver’s license)
• Your insurance card
• Your method of copayment or payment in full if your visit will not be covered by insurance
• List of current medications and supplements you are taking, including name and dosage
• Any meters, insulin pumps, continuous glucose monitors and/or glucose logs that provide details about your insulin and sugar levels
