Contact CoreLife’s patient navigators at 336-993-7875 to learn more or schedule your new patient appointment.  As part of the intake process, our team will verify your insurance coverage prior to your first visit. You can make an appointment with or without a referral from your primary care provider.

We ask that you bring the following items with you:

• Your insurance card
• A method of payment, to cover your copay
• A form of photo ID
• A list of the medications and vitamins you take regularly
• A record that describes your medical history, including surgeries
