If you are already a patient of this clinic, you can use MyChart or call during office hours to schedule an appointment.
New Patients
You need a referral from your primary care provider to be seen in this clinic. You can find one on our Providers Page.
You can use MyChart or open scheduling through our Providers page to schedule a visit online, or call us during office hours so we can assist you.
While our clinic does not require a referral, your insurance might require one for your visit to be covered. Please check coverage before scheduling.
What to bring
We look forward to serving you. Please arrive at least 15 minutes before your visit. If you arehaving a treadmill test, bring or wear comfortable clothing, including shoes.
All patients should bring:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
