Your Visit
We are a referral-based clinic. Once a provider involved in your care makes a referral to our clinic, we will contact you within 24 hours of receiving your referral information to schedule your first visit.
You can use MyChart to schedule a follow-up online anytime, or call our clinic during office hours so we can schedule for you.
If you are seeking a second opinion, please call our clinic during office hours.
New Patients
We look forward to being a part of your care. Please arrive 30 minutes before your first visit so we can get needed information from you.
What to Bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
