Appointments
We are a referral-based clinic. Once a provider involved in your care sends your referral information, we will contact you to schedule a visit.
If you are an established patient and need to schedule an appointment, please contact us on MyChart or call us during office hours.
New Patients
We welcome new patients. In order to make your first visit as productive as possible, we ask that you arrive 15 minutes before your first scheduled appointment so we can collect any needed information from you.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
