Appointments
Appointments are scheduled through our Winston-Salem office. Call during business hours to schedule your visit for any of our three locations: Mount Airy, Kernersville or Winston-Salem. Same-day appointments are often available if you have an urgent medical problem.
Please arrive at least 15 minutes before your appointment time. Call at least 24 hours in advance to cancel or reschedule your visit.
What to bring
Please bring the following items when you check in for your visit:
- Insurance card
- Photo ID
- List of your prescription and over-the-counter medications, as well as information about your dose size and frequency of use
- Details about your medical and surgical history
New patients
We look forward to welcoming you on the day of your first visit. Before calling to schedule an appointment, please contact your primary care provider and ask for a referral. You do not need a referral if you are transferring from another brain and spine clinic; please contact your neurologist or neurosurgeon and ask to have your medical records transferred.
Patient forms
You may print and complete the forms below and bring them with you to your appointment. We can also have them available for you to complete when you check in at our clinic, if that would be more convenient.
