Appointments
You can use MyChart or open scheduling on our provider page to schedule an appointment right now. You can also call our clinic during office hours so we can schedule an appointment for you.
If you need to cancel or change an appointment, please call at least 24 hours in advance. Failure to show up or cancel your appointment will result in a missed appointment fee that will need to be paid before your next visit.
Attending appointments is crucial to your care. After three missed or canceled appointments, you might be dismissed from our clinic.
What to bring
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- All medications you are currently taking in their original bottles
- Any forms you have completed ahead of time using the links below
