Appointments

You can schedule a visit by calling our clinic during regular business hours, Monday through Friday, 8 a.m. to 5 p.m. Please have information about your health insurance plan available when you call so we can help you determine coverage for your visit.

You will receive a reminder call from our clinic one to two days before your appointment. If you need to reschedule or cancel your appointment, please call our clinic 24 hours in advance. If you call after our normal business hours, your call will be answered by our after-hours service.

What to bring

Please bring your insurance cards, as well as a list of your current prescriptions and over-the-counter medications, including herbal or nutritional supplements. This should include information about doses and frequency of use. Any of these could have the potential to cause a reaction to other medications we may prescribe.

New patients

Please arrive 15 minutes prior to your scheduled appointment to ensure all paperwork is complete.
