To make an appointment to see a provider in our clinic, you can use MyChart or open scheduling through our provider page to schedule online right now. You can also call our clinic during office hours.

What to bring
- Your photo ID
- Your insurance card, or the insurance card covering your child if your child is the patient
- Your copayment or payment in full
- A list of medications you are or your child is currently taking
