Appointments
You can make an appointment to see a provider in our clinic now using MyChart or open scheduling through our provider page. Please call our office during business hours to schedule appointments.
We are open weekdays with the exception of following holidays:
- Good Friday
- Memorial Day
- Independence Day
- Labor Day
- Thanksgiving Day
- Christmas Day
- New Year’s Day
We look forward to seeing you on the day of your appointment. Please arrive at least 15 minutes before your first appointment. When you arrive for your appointment, please remain in the car and call our check-in line at 336-673-6335.
What to bring
- Your photo ID
- Your insurance card, or the insurance card covering your child if your child is the patient
- A payment to cover your copay or payment in full if you are responsible for the costs of your visit
- A list of medications you are or your child is currently taking
- Forms you have completed ahead of time using the links below
