When you leave the hospital, you may need to stay in a rehabilitation or skilled nursing facility (SNF) or get help at home from a home health agency. This is called “post-acute care.”
Post-Acute Network
Novant Health collaborates with a select group of skilled nursing facilities (SNF) and home health agencies that we call our “post-acute network.” Participants in this network are committed to working with Novant Health to improve patient outcomes after discharge from the hospital, and to focus on what we can do together to prevent hospital readmissions. Facilities in this network also agree to measure overall quality and performance and maintain a high Centers for Medicare and Medicaid star rating.
By selecting a provider in the post-acute network for your after-hospital care, you know that:
- You have the right team to help meet your unique needs
- Your Novant Health providers will work with your post-acute providers to create a personalized care plan
- Communication between providers involved in your care will be improved
- The focus will be on delivering high-quality care while in a skilled nursing facility or working with a home health provider
